Definition
A required human approval before an automated system takes an irreversible action — sending, publishing, charging, committing. The difference between a system that fails safely and one that fails publicly.
Gates should sit at irreversibility, not at difficulty. Drafting needs no gate; sending does. Scoring a lead needs no gate; emailing that lead a price does.
The common failure is a gate everyone clicks through. If a reviewer approves 200 items a day they are not reviewing, and the gate is theatre. Either sample properly or reduce the volume reaching the gate.
